Need help with php email submit box

I'm trying to make a one form submit box for submitting an email. 
when they click submit, an email will be sent to my chosen email, but the message doesn't need to contain anything.
I want the email that is submitted in the form to show as the sender of the message. I can't get the right thing to show in From header.
Could someone show me some example code?
sorry if I'm not making any sense, not to familiar with this stuff.
